About our Organisation

Founded by two health professional with vast experience in the health and disability sectors, Dahlia Homes is a registered NDIS provider operating in the Australian Capital Territory (ACT) and New South Wales (NSW).

Established due to our commitment to the care we have for the people in our communities, Dahlia Homes will be providing specialist disability services to the community members living with disabilities, irrespective of cultural background, gender, age, religious affiliations and health conditions.

Services Offered

At Dahlia Homes we provide a safe, warm, participant centred care in a comfortable environment. our Group Homes will have 2 to 4 participants and we offer high standard of service. We have highly qualified staff to provide wholistic care to our participants and they can handle participants with complex needs. Services are tailored to meet the unique requirements of each individual. Our group homes will be located both in ACT and NSW regions.

Community Nursing Services

At Dahlia Homes we will be providing nursing services in the community to clients with complex needs and therefore we have a team of qualified Registered Nurses and Allied Health Professionals. Care plans are made in consultation with the clients to meet their individual needs. Services provided include the following:

  • Skilled nursing care
  • General nursing care
  • Medication administration & management
  • Colostomy and Catheter Care
  • Wound management
  • Liaising with relevant stakeholders (GP’s)
  • Follow-up care after hospital admission
  • Assistance with chronic illnesses such as diabetes and epilepsy management.
  • Continence aids assessments and recommendations & in-house training by a nurse
Care
Transport

Transport

Dahlia Homes will be providing transport services to our clients and other clients in the community. This will enable our clients to attend to activities and appointments to live a fulfilling life while in the community.

Respite Care

At Dahlia Homes we understand that everyone needs a break. We will be offering respite care on short term basis ranging from few hours, days or weeks in a friendly environment the choice is yours.

Who is the National Disability Insurance Agency?

The National Disability Insurance Agency is an independent agency that has been set up to deliver the NDIS. The NDIA is overseen by the Council of Australian Governments Disability Reform Council. The NDIA website contains information for people with disability, including the NDIS Access Checklist, a tool which helps people to determine if they meet the eligibility criteria.

If the National Disability Insurance Scheme is available in your area, the first step you need to take is to find out if you meet the access requirements, which are available on the national NDIS website.
